New Patient Application

Thank you for your interest in receiving care with us. Our waitlist is currently several months out, but we would love to connect with you. Please fill out our New Patient Application so we can reach out and schedule your first appointment as soon as possible.

Patient Forms

To ease your arrival please digitally complete the 1 intake form specific to your service and email it back within 2 weeks of your 1st appointment. Please download this to your computer first, fill out the form fields, save the PDF and then email to abelchiro.team@gmail.com.

Financial Policy

Abel Chiropractic accepts cash, credit card, checks, flexible spending, and HSA accounts to pay at time of service.

We are an out-of-network provider and do not file insurance. We will provide documentation for you to submit to your health insurance provider. It is best to contact your insurance company to find out what their process is for claim submission and reimbursement.

A good faith estimate is available for those who are uninsured or cash patients. Medicaid is out of pocket pay. Click the link below to learn more.

Cancellation Policy

At Abel Chiropractic, we emphasize high quality chiropractic care and health services with appointments reserved specifically for you. Due to our dedication to your health care, we often have a waiting list for appointment times. If you cancel on short notice, do not show up, or show up very late-it is a lost opportunity for another patient.

We understand unanticipated events happen occasionally in everyone’s life and we will take that into consideration, however in our desire to be fair to all patients and maintain a viable practice, the following policies are honored.
